The dataset spanned 31 different vessel types, with separate detailed analyses conducted on bulk cargo vessels, container ships, tankers, and superyachts. For each of these, the top three contributory factors were identified across four levels of the SHIELD taxonomy: Acts, Preconditions, Operational Leadership, and Organisation.

Although many sectors often view their challenges as unique, the findings suggest that the underlying human factors are almost identical, making safety lessons highly transferable across industry sectors. This consistency indicates that the root causes of safety incidents are not limited to specific vessel types or operations. They are systemic and often originate ashore, rather than on board the vessel itself.

SHIELD Category

All vessels

Bulk Cargo

Container vessels

Tankers

Superyachts

Description

Acts

AP1

AP1

AP1

AP1

AP1

No/late visual detection

AD1

AD1

AD1

AD1

AD1

Incorrect decision or plan

AI1

AI1

AI1

AI1

AI1

Workaround in normal conditions

Preconditions

PTG2

PTG2

PTG2

PTG2

PTG2

No cross-check and speaking-up my team member

PEW2

PEW2

PEW2

PEW2

PEW2

Technology creates unsafe situation

PPF7

PPF7

PPF7

PAW4**

PPF7

Risk underestimation

Operational Leadership

LO1

LO1

LO1

LO1

LO1

Inadequate risk assessment

LT1

LT1

LT1

LT1

LT1

Inadequate supervision

LT2

LT2

LT2

LT2

LT2

No correction of unsafe practices

 Organisation

OS2

OS2

OS2

OS2

OS2

Safety risk assurance

OS3

OS3

OS3

OS3

OS3

Safety risk assurance

OR5

OR5

OR5

OC1**

OR5

Design of equipment or procedures

**The top 3 factors in each of the SHIELD categories were identical across all vessel types with the exception of tanker vessels, which showed minor variation in preconditions and at the organisational level.

Organisational Decisions: The Hand on the Tiller of Safety

At the organisational level, decisions made ashore contributed to many of the most persistent safety issues. Procurement of poorly or inadequately designed equipment was very frequently cited as a contributory cause in incident reports. Examples include pilot transfer arrangement designs that could not comply with regulations; poorly placed or missing attachment points for working aloft safely; or sub-optimal mooring bollard locations that compromise line handling.

In many cases, risk assessments were either not conducted or failed to identify critical hazards, particularly those arising from the interaction between people, equipment, and the environment. Even when documentation existed, it was often insufficient to support the safe execution of tasks.

However, underlying many of these failures is a critical and often uncomfortable truth: hat resources (financial, human, and technical) are not always allocated in a way that supports safety as a strategic priority.

Safety cannot be delivered on intent alone. Board-level decisions that underfund maintenance, delay crew change cycles, or prioritise operational efficiency over fatigue management will ultimately ripple down to the deck plates. When budgets are tight, it is often training, supervision, and procedural development that are quietly sacrificed, despite their essential role in risk management.

Importantly, this is not a problem that organisations face in isolation. Regulatory bodies, flag states, classification societies, and port state control authorities set the baseline conditions for maritime safety. While these frameworks are designed to safeguard operations, incident reports have shown that compliance with regulations does not always translate into real-world safety.

Several reports referenced equipment or procedures that passed inspections yet proved unsuitable in practice. This disconnect highlights that compliance does not always equal safety. Regulatory inspections often focus on documentation and visible conditions rather than on usability, human factors, or the real operational environment. Design standards, too, can fall short when they don’t fully consider how equipment is used on board.

Moreover, regulators often react to incidents rather than anticipate latent risks such as understaffing, fatigue, or alarm overload; all of which are factors that precede unsafe acts. To close this gap, regulators must move beyond enforcing minimum standards and towards enabling resilient, human-centred safety frameworks that involve end users in setting standards and emphasise learning over blame.

When organisations and regulators align by integrating safety as a shared priority rather than a checklist, the ripple effects are clear. Investments made in the boardroom, supported by forward-thinking regulation, create resilient conditions that support safe operations on board.

As Captain John Lloyd, CEO of The Nautical Institute, has noted:

“Many accidents that occur on ships have their roots in decisions made far from the vessel, often in the boardroom.”

The CHIRP analysis reinforces this perspective: seafarers are better supported in managing the complex, high-risk environments they operate in when safety is embedded into budgeting, procurement, staffing levels, vessel design, and procedural development.

Too often, safety is seen as a cost centre rather than a core enabler of continuity, reputation, and performance. But the evidence is clear: investments made in the boardroom are felt directly and immediately on the bridge, in the engine room, and on deck in terms of safety outcomes.

Operational Leadership: the critical layer of influence

The analysis highlights the critical role of operational leadership in translating organisational intent into safe and effective action on board. This is the level at which strategic decisions such as risk tolerances, procedural expectations, and resource allocations are either enabled or hindered by the support structures put in place ashore.

Across vessel types, the data reveals recurring challenges: operational risks are not always fully evaluated before tasks begin; supervision may lack the depth or presence needed to match the complexity of the operation; and known unsafe practices are sometimes left unaddressed, even when observed.

These issues often stem not from indifference but from a lack of resources and empowerment. Shipboard leaders are frequently left managing complex operations with:

limited crew numbers and skills mix, which stretches safe manning beyond sustainable limits.

incomplete or outdated procedures that do not reflect actual work conditions.

conflicting pressures to maintain commercial schedules despite known fatigue risks.

training that focuses on compliance rather than critical decision-making or human performance.

Whether through cost-saving decisions, inefficient structures, or unclear accountability, when organisations restrict access to these critical enablers, they create conditions in which even the most committed onboard leaders can struggle to maintain a safe operation.

The boardroom may never see the moment when a fatigued officer is expected to oversee a high-risk task shorthanded, or when a supervisor lacks the confidence to stop a job due to ambiguous procedures. But those moments are the downstream impact of upstream choices.

To close this gap, investment in leadership must extend beyond titles; it must encompass the time, tools, training, and trust necessary to act decisively. A strong safety culture does not trickle down on its own; it must be actively supported from the top, with resources that reflect the realities of operational life.

Preconditions: the conditions that shape performance

A consistent theme across vessel types was the absence of robust cross-checking and challenge within teams. In many cases, critical tasks were carried out without a multi-person approach or open communication of concerns. This is rarely due to cultural shortcomings alone—it often reflects the reality of limited crew numbers, time pressures, and insufficient training in assertive communication. These conditions are not accidental; they result from upstream decisions regarding manning models, training investments, and task allocation.

Complacency also emerged as a recurring factor. When operations are stripped to their most efficient form—with minimal redundancy, no time for structured reviews, and high workloads—complacency isn’t just a human failing; it becomes an organisational symptom. Crew members are often left to perform complex tasks under the assumption of normality, with little bandwidth for proactive challenge or reflective thinking.

In addition, the design and performance of technical systems were frequently cited as contributing factors. Unintuitive interfaces, alarms that are poorly prioritised, or automation that behaves unpredictably can all degrade human performance, especially under pressure. Too often, these design flaws are inherited from procurement decisions made with little end-user input or selected based on short-term commercial factors rather than usability or maintainability.

Acts: the final link in the chain

These are the most visible moments in incident reports, but they are rarely isolated failures of individual judgment. Instead, they are often the result of accumulated pressures, unclear procedures, or cognitive overload in complex environments.

In some cases, the operator may not have had access to the correct information at the right time. In others, the decision-making environment may have been shaped by fatigue, distraction, or conflicting priorities. These are not abstract conditions; they are often the result of tight rotations, deferred maintenance, overlapping responsibilities, or under-resourced departments ashore. When organisations stretch their people thin, they increase the likelihood that the last link in the chain will fail.

We must stop treating unsafe Acts as the origin of failure. They are the last visible symptom of invisible constraints placed much earlier in the system, often long before the vessel left port.

A Call to Action: safety starts at the top

The consistency of findings across vessel types and over a five-year period should be a Red Flag for industry. The human factors contributing to safety incidents are not anomalies, they are systemic and shaped by the structures, decisions, and resourcing strategies within organisations.

This is not a call to fix individuals. Rather, we should follow the advice of Professor Sydney Dekker, the author of “Safety Differently”:

“We should not be trying to fix the human but rather fix the system to account for human limitations.”

It behoves us all to ask ourselves:

are our risk assessments identifying the hazards that crews actually face, or only those that fit into policy frameworks?

are our procedures reflect the lived realities of shipboard life, not just ideal conditions imagined ashore?

do we give our operational leaders the time, tools, and training to manage risk, not just maintain output?

are budgetary decisions made with a clear understanding of their downstream safety impact?

are we focussing on safety, or merely compliance?

","post_title":"From Boardroom to Bridge: the Human Factor thread in Maritime Safety","post_excerpt":"","post_status":"publish","comment_status":"closed","ping_status":"closed","post_password":"","post_name":"from-boardroom-to-bridge-the-human-factor-thread-in-maritime-safety","to_ping":"","pinged":"","post_modified":"2025-09-08 09:38:07","post_modified_gmt":"2025-09-08 08:38:07","post_content_filtered":"","post_parent":0,"guid":"https://chirp.co.uk/?post_type=hot-topic&p=35011","menu_order":0,"post_type":"hot-topic","post_mime_type":"","comment_count":"0","filter":"raw"},{"ID":34817,"post_author":"5","post_date":"2025-06-30 13:30:08","post_date_gmt":"2025-06-30 12:30:08","post_content":"

The findings of a focused survey of UK seafarers providing insights into the experiences of individuals relating to sexual offences at sea has been published. Despite significant safety measures, accidents still occur, prompting a continuous quest for improvement. This article delves into the utilization of automated vessel self-cleaning systems to mitigate risks and enhance safety. We explore the journey of implementing these systems, overcoming challenges, and reaping the benefits they offer.

Enclosed spaces pose substantial risks, and traditional safety measures have their limitations. Acknowledging this, industry professionals sought alternatives to minimize the need for human entry into tanks, where dangers lurk. The realization that the best way to reduce risks is to avoid putting people in harm’s way led to the exploration of automated vessel self-cleaning systems.

Implementing this new approach required concerted efforts from various stakeholders. Vessel operators, crews, and logistics providers were engaged in discussions to promote and implement self-tank cleaning. Overcoming resistance and changing entrenched mindsets proved to be a challenge, but the support and cooperation of these key players were crucial in driving the adoption of this innovative solution.

Automated vessel self-cleaning systems employ spinning nozzle heads that eject high-pressure water in all directions within the tanks, effectively breaking down residues. The use of water and detergent, akin to established COW and Butterworth nozzle technologies, ensures efficient cleaning. Dosing tanks allow for adjusting the detergent amount, while heated water at around 40 degrees Celsius maximizes the cleaning effect. Tank cleaning cycles can vary in duration, depending on the tank’s condition and the desired cleanliness standard. Addressing the challenge of timely discharge/ stripping pump operation to prevent solids settling while avoiding pump dry runs has been a significant aspect of system optimization.

The adoption of automated vessel self-cleaning systems yields numerous benefits. First and foremost, it reduces the need for personnel to enter tanks, effectively mitigating risks associated with confined space entry. Furthermore, it minimizes the occurrences of slips, trips, and falls, reduces working at heights, and decreases exposure to hazardous chemicals. Cost reduction increased operational efficiency, and improved quayside congestion are additional advantages, as self-cleaning allows tanks to be cleaned at sea or while in port. The generation of similar or less waste, elimination of scaffolding requirements, reduced vessel movements, and a smaller carbon footprint further enhance the appeal of these systems.

To further reduce the need for human entry into confined spaces, recent trials have involved the use drones for remote tank inspections. These provide a rapid and safe means for remote assessment and provide much greater fidelity of data than previously achievable, surpassing even that of reach-pole cameras (see here for an example: https://www.youtube.com/watch?v=Dl5qebdnO8M ). This innovative approach demonstrates promising potential for reducing reliance on physical tank inspections and improving safety protocols. In the pursuit of improved safety standards, automated vessel self-cleaning systems have emerged as a game-changer in the shipping industry. By minimizing the need for personnel to enter tanks and leveraging advanced technologies for remote inspections, risks are mitigated, costs are reduced, and operational efficiency is enhanced. With the continued support and collaboration of industry stakeholders, the widespread adoption of these systems can revolutionize safety practices, ensuring a safer working environment for all As fishing is notoriously one of the world’s most dangerous professions2, recognition of these links is also embedded in the ILO’s (2007) Work in Fishing Convention (c188) – which establishes minimum standards for decent work on board fishing vessels3. Decent work is just one end of the spectrum of working conditions on board fishing vessels, with egregious violations of human rights constituting forced labour, human trafficking, and modern slavery at the other end. In between decent work and forced labour are a range of conditions that may be exploitative and discriminatory but not in violation of labour laws (e.g., unequal pay for migrant fishers for equal, shared work with national fishers) or conditions that violate labour rights and protections, but may not amount to forced labour)4.

Bidirectional linkages between (un)safe working conditions and (in)decent work across fleets globally have also emerged in research. First, exploitative labour practices make the work on board vessels even more unsafe. For example, many exploited fishers working in fleets from Thailand5 to the UK4 to China6 report excessive working hours in contravention of ILO C188; denial and sometimes falsification of rest hours; and tied immigration schemes that blur the lines of what constitutes work – compelling some fishers to perform unvalued work (e.g., mending nets and vessel repairs) onboard the vessel while in port on their ‘rest’ days, or this may involve the denial or withholding of food and water until a certain amount of fish has been caught; as punishment for a poor catch.

Both scenarios compound the dangers already involved when working on a vessel as fatigued and malnourished fishers are more prone to making mistakes with serious consequences to their health and safety, and potentially the health and safety of others on board the vessel through no fault of their own. In research from the UK, migrant fishers in the sample were significantly more likely to incur injuries than national fishers4. Further, since many exploited fishers globally are transnational migrants, their precarious
immigration status may deny them access to medical care, including routine medical care that could offer early detection of illnesses associated with extreme and chronic fatigue and malnourishment.

Safety matters may also influence working conditions. There is some speculation, though it has yet to be empirically tested, that safety violations may be an early indicator of future exploitative labour practices, as these violations may be an early warning sign of a tipping point into decreasing profitability and the associated ‘corner cutting’ that often underpins the exploitation of crew7. And increasingly, the fishing industry needs to anticipate and plan for future scenarios where climate change will also likely exacerbate these links between safety and decent work, such as extreme storms, extreme heat, and wave and wind changes that may lead to occupationally hazardous work, longer trips at sea, longer working hours, and the need for more safety equipment. Suppose these impacts are not mitigated, and the industry is perceived as becoming more dangerous due to climatic changes. In that case, it could intensify crew labour shortages that are known to increase reliance on migrant fishers and drive exploitative practices.

The industry must also grapple with how to understand and frame these interconnections. On the one hand, contextualising working conditions within a more significant reference of safety offers the potential for greater stakeholder buy-in as it is frequently less divisive of a topic than the treatment of migrant crew. On the other hand, such contextualisation may also risk overlooking the systemic drivers of fishing crew exploitation and fair washing exploitative practices that do not reach the threshold of forced labour as decent work.

Fatigued and malnourished fishers are more prone to making mistakes with serious consequences to their health and safety

Dr Jess Sparks is a Research Assistant Professor at the Friedman School of Nutrition Science and Policy at Tufts University and a Research Fellow at the University of Nottingham Rights Lab.
